Financial Policy

Thank you for choosing us as your Health Care Provider. We are committed to your treatment being successful. The following is a statement of our Financial Policy, which we require you read and agree to prior to any care. The purpose of our financial policy is to keep HEALTH CARE COST DOWN. All patients must agree to our entrance forms and financial policy in their entirety before an appointment will be scheduled.

Payment of Services

Payment in full is due at the time of service. Please note all services are to be paid in full at the time of your visit. All service and sales are final. For your safety, all supplementation sales are final. We do not allow any returns on supplements due to inability to control the temperature and handling of these products once they are leave our control. Improper handling may damage the product rendering them less effective or ineffective.  If a supplement arrives damaged or in a condition that does not meet standards, the clinical will work with you to replace the supplement. All lab testing fees are final after a lab specimen has been submitted to the lab.

Payment methods are limited to credit cards (Visa, MasterCard, American Express and Discover).  We do require payment to be made at the completion of each visit.  No delayed payments will be accepted.

Notification of No Insurance Acceptance

We do not accept assignment of insurance benefits.  Your insurance policy is a contract between you and your insurance company. We are not a party to that contract. We as a clinic will not assist you in seeking reimbursement for your insurance company as the doctor nor clinic has a role in this relationship.  We will not make copies of records or submit documentation directly to insurance companies.  Again, we are not a party to your insurance contract.  Please be aware that some, and perhaps all, of the service provided may be non-covered and not considered reasonable or medically necessary by your insurance policy.

Usual and Customary Fees

Our practice is committed to providing the best treatment for our patients and we charge what is usual and customary for our area. You are responsible for payment regardless of any insurance company or other organization’s arbitrary determination of usual and customary rates.

Services Offered

The clinicians of Genesis Wellness offer many services to support the needs of the patient.  During the course of treatment, we may offer interventions to address your complaints and the factors that are deemed to be associated with their development, including, but not limited to internal dysfunction and nutrient deficiencies.  All of the care offered has shown to be safe, efficacious and to have clinical benefit.  Understanding this, the FDA and other governing organizations continue to deem some of the procedures and intervention options used as experimental and/or investigational.  By agreeing to this form you confirm that you have reviewed this fact and are aware that this may be the case with some of the intervention options that are considered appropriate for you during your course of care.  Explicitly, you are giving informed consent for the care recommended.

X-rays, MRI, Ultrasound 

All x-rays / MRIs ordered by the doctor are referred to an outside diagnostic imaging facility, thus all imaging concerns should be directed to that facility.

Missed Appointments

All visits must be canceled at least 48 hours in advance to avoid a “Failed To Show” charge being charged for your missed appointment time-slot. Please help us serve you and others better by keeping your scheduled appointments.

Phone / Virtual Consultation

We understand that phone / virtual consultations provide a method of receiving essential healthcare information to seek and continue care in a timely manner without the inconvenience of travel and lost time.  As a convenience for our patients, all visits with Genesis Wellness are completed in this format.  As this option becomes more mainstream, we want to assist you in obtaining the most from your visit by providing you with expectations that will aid you in structuring your interaction with your doctor.

The phone / virtual interaction is in many ways similar to a traditional visit.  This is the time for you to receive answers to your questions and share medically relevant information that is pertinent to your care.  These will help the doctor better understand your needs to make the best decision for you.  Equally, phone / virtual consultations are scheduled based on an allotted time of 1 hour for new patients and 30 minutes for existing patients.  During this time, patients should expect to be able to express their needs and conclude the visit with a precise and actionable item(s) to further their care.

As should be expected, there will be new options associated with phone / virtual consultations as well.  Most of these have to do with the follow up communication process from the doctor and staff.  As this visit is not taking place in-office, the ability to provide resources related to your care will require an active email address that is regularly checked.  This will be the location that you will receive items such as updated dosing information, appointment reminders and electronic resources.

Additionally, the checkout process will take place online with processing securely through Authorize.net.  If you wish for a card that is different than the one kept on file to be charged, it is your responsibility to communicate this to the office at the time that your visit is confirmed.  Should you ask for a refund related to charging of the card on file and requesting a different card to be charged, you will incur a 5% fee based on the associated charge.  For example, if your request that $140 be refunded to your card, you will incur a fee of $7. Additional items that may warrant a charge such as lab testing or nutritional supplements will NOT be charged without electronic authorization through email.

PMA Introduction

Families all over the world are waking up to the fact that conventional medicine is “sick care” and while sick care is a blessing for life threatening disease and emergencies, it is not the best choice for health improvement or routine and chronic conditions.  As more and more families realize this truth, they are speaking out in a big way, demanding fewer drugs and more nutritional, dietary and lifestyle alternatives as a better way, a safer and healthier way!

Safe, natural health care is not intended to
replace conventional medicine.  It is simply
a better choice for building better health, for
non-emergency, routine and chronic illness!

But there is a problem.  Families that decide to turn to natural alternatives often find it difficult and frustrating to locate proven information or a reputable professional practitioner they are comfortable turning to for their family’s health.  There is however an understandable reason why quality natural health is difficult to find.  Government regulatory boards are charged with protecting the public and often make it very unpleasant for practitioners that offer non-conventional approaches.  While public protection is certainly a worthy concern, and as well intended as these regulatory authorities may be, unfortunately the individual and family that seeks a natural approach to health is the big loser because fewer professional level natural health services are available.  Moreover such practitioners are not allowed to recommend the very therapies that they were trained in through their professional education and post-graduate training.

If your doctor has seemed hesitant to discuss natural alternatives, or you find your natural health practitioner essentially hiding in the shadows, keeping a low profile with a small client base, now you know why.  Another impediment to finding a professional level of natural services is the fact that practitioners are not typically regulated, which means they are not accountable for their services and this goes even for practitioners in states that have passed health freedom legislation.  While “too much” regulation is bad, no regulation is likely much worse.  From over control to no control!

Fortunately, there is a solution to these problems. The Pastoral Medical Association (PMA) Member Share Network was developed to bring a balance, to address these issues and to provide access to a professional level of natural alternatives for you and your family.  Natural medicine is God’s medicine and due to Constitutional provisions, PMA network practitioners are protected from regulatory boards that would otherwise restrict them from assisting you.

Health care practitioners, whether state licensed or not, that seek license to offer services in the Network must pass rigid standards to qualify for PMA license.  This means that as your practitioner, Dr. Hill is being held to a higher standard beyond just the state licensing board.  He must meet and maintain specific graduate and post-graduate educational standards, must participate in a comprehensive evaluation of his or her credentials and practice as well as pass a detailed background investigation and participate in at least two interview sessions with a PMA license representative. Practitioners spend significant dollars in the license application process and their annual license fees average a couple thousand dollars.  In other words, the PMA license process is more stringent and costly than many state license boards.  PMA licensees must not only meet the educational, continuing education and licensing requirements of their initial and currently held degrees, but must also meet the criteria of the PMA.  As you can see, practitioners that succeed through the PMA license process are clearly dedicated practitioners who are very serious about their practice and services to you.  It should be no other way!

The designation D.PSc is used by Dr. Hill.  The designation of D.PSc or PSc.D means that a practitioner is PMA licensed and meets Quality and Professional Standards that sets them apart when providing natural health services.

While the PMA Network, not unlike state agencies, cannot guarantee that any provider or the services they provide are right or suitable for any individual, you can rest easy knowing that the Network is populated with verified professionals in all U.S. States, throughout Canada and multiple other Countries and this includes mental health counselors, medical doctors, pastors, chiropractors, natural health professionals, acupuncturist, herbalist, and many more trained professionals.  As a practitioner, it is my obligation to disclose my qualifications to you, only provide services I am trained and qualified to offer and will discuss my proposed plan of helping you and the expected outcome before beginning service to you.

How the Process Works with Our Clinic

As part of the new patient intake process, you will need to complete the online personal health form.  Filling out this information is a routine part of a doctor visit, however our paperwork has two unique differences.  First, you will notice that the intake process is a little longer than what is customarily used.  This is intentional as we want to know as much about your health as you can tell use to enhance the decision-making process when it comes to your care.  Second, you will see an agreement to join the PMA.  As noted, the PMA is the agreement whereby I am able to offer you the nutritional, dietary and lifestyle recommendations that you seek to improve your health.  Moreover, it is my legal obligation to make you aware of this.  We understand that this is different than what you are used to and realize that you may have questions.  These will be addressed below in the FAQ section.  However, you have not sought out this type of care because you are looking for the same routine sick care approach that is commonly used.  Rather than ignore the core concepts of health that help you achieve wellness, completing the PMA agreement allows you unrestricted access to the years of knowledge and training that Dr. Hill has acquired.

FREQUENTLY ASKED QUESTIONS ABOUT THE PMA

Why am I signing a PMA agreement?

The Pastoral Medical Association (PMA) is a medical association that allows for the practicing of those sciences that a practitioner has been trained in and proven competency that may not be otherwise covered by another board.  For the protection of you as the patient, it is important that all therapies recommended by a practitioner have oversight from a board.  In the case of Dr. Hill’s practice, this allows him to use nutrition and lab testing to the fullest extent of his education, research and knowledge base.

You are agreeing to the PMA to show that you acknowledge and are comfortable with the credentials possessed by the practitioner that you will be seeing.

Will I be asked to join or receive anything from the Pastoral Medical Association?

The PMA is not looking for membership from the public. It is an oversight organization whose members are the healthcare practitioners that carry its professional designation.

Will I be discussing religion in my appointments?

You will ONLY be discussing your health and those options related to better defining or improving its state.

Will I have any new religious affiliations by signing the PMA agreement?

The PMA is a means to help you as the healthcare consumer have freedom of choice.

What can I expect if I do not sign the PMA paperwork?

So, that Dr. Hill is able to make the best decisions for your healthcare, we ask that all patients sign the PMA agreement.  In cases where those that wish to be patients do not sign the PMA agreement, we understand fully.  Unfortunately, due to the ever-changing medico-legal climate, practicing within the letter of the law has never been more crucial.  Therefore, rather than taking the chance of recommending a therapy that would be outside of the jurisdiction of the PMA, we refuse the right to accept a new patient should the circumstance present.

Will the PMA enhance the level of care and options that Dr. Hill can offer?

YES.  Dr. Hill will be able to more freely recommend nutritional, dietary and lifestyle recommendations.

Will the PMA ever be brought up again in a consultation with Dr. Hill or by the staff after it has initially been signed?

We are here to help you meet your healthcare needs and that will be the sole focus of conversation with Dr. Hill and staff.

Why use an ecclesiastical affiliation rather than setting up a state or national organization?

An ecclesiastical, or church, related body is independent of the state based on the notion of separation of church and state.  Therefore, a practitioner is free to practice those therapies for which they are competent and trained, so long as they are deemed safe, without interference from a restrictive state organization.  For example, in the case of Dr. Hill, who holds active diplomate status with the American Board of Clinical Nutrition, he is free to make nutritional recommendations in the interest of the patient’s health so long as they are safe.

Is the PMA associated with a specific church?

There is no denominational or church affiliation with the PMA.
